• This plan is available to active EUTF and active HSTA VB participants who are covered under a private sector or federal government group health plan.
• The plan reimburses you for eligible, out of pocket, co-payments you pay for medical, hospital and surgical expense.
• Your private sector or federal government health plan must cover the expense first.
• Co-payments on prescriptions drugs are eligible for reimbursement.
• All reimbursements are made directly to you.
• Please refer to the policy or certificate of insurance for details, limits and exclusions.
